If even then the tape not coming out then that is surely a hardware error and you need to check with the hardware vendor for the same.

I don't think I would have told BE to format tapes as LTO doesn't need formatting. The engineer has said he'll talk to his manager and get back to me. To them if the tape hasn't ejected they have no way of knowing if the backup ran or not so call us.
